INTRODUCTORY ADDRESS OF THE CHAIRMAN In line with the economical situation worldwide, we have seen in 2008 a decline in projects and initiatives. Fortunately 2009 made good for it . During a brief visit of DEMMA members to Mytishchy in February 2009 a new Agreement was signed by DEMMA Chairman Paul Mulkens and the newly appointed Chairman of DEMMA-M Wladislav Mirzonov. A sound basis was formed for further exchange programmes in 2009, 2010 and 2011 During a visit to Erisman Federal Science Centre of Hygiene, opportunities were explored to extend liason with the DEMMA Foundation. We also saw retirement from DEMMA staff and beause of completion of the Healty Lifestyle Project. Resulting in a farewell and thank you to four members: Aleid Tensen, Gottlieb Nelck, Wim Eltink and Loek ter Beek. The Committee of Recommendation welcomed a new member Mr. Jacques Tichelaar, Queens Commissioner of the Province of Drenthe. A new website www.demma.nl was introduced, allowing us to inform a wider circle of people about DEMMA’s activities. Finally a congratulation for DEMMA-Mytishchy Secretary Valery Yanin, who was elected in the Mytishchy City Council and charged with the portfolio of Health Care. 2009 therefore turned out to be a good year for DEMMA.

MEETINGS, PROGRAMMES and EXCHANGES in 2009 Meetings Demma have met five times in 2009, namely on 26 th of January 10 th of March 18 th of May 5th of October and on 21st of December For the various visits and exchanges, dedicated Working Group were established. Their meetings were at hoc and not minuted.

Programmes and Exchanges A delegation from DEMMA visited Mytishchy from 25-29 February 2009 to discuss, agree and sign a new Agreement with DEMMA-Mytishchy. In November 2009 a simultane visit took place in Emmen; Dr Alexander Aliev, a neurologist from Erismann Health Institute in Mytishchy and Dr. Alexander Lebedev, cardiologist of Mytishchy Hospital stayed three weeks in Scheper Ziekenhuis for what is regarded as a traditional Demma Exchange programme. In the last week of the three week stay they were accompanied by Dr.Vladislav Mirzonov, the new Chairman of Demma in Mytishchy and Dr.Valery Yanin, Secretary The latter two were introduced to the aspects of Hospital Management in The Netherlands, in a parallel programme The combination of the two twin visits was named Double Dwoika thereafter. ( Dwoika= a two horse sleigh; dwa=two) Also in November, DEMMA-M organised a Symposium on General Practice in Mytishchy. Coen Holzhauer made a presentation and Guus de Groot also attended. 2

NEW AGREEMENT WITH DEMMA-MYTISHCHY A summary of the new agreement 1. To increase the value of an exchange program, more emphasis will be put on the acuity of English of visitors Focal points: Guus de Groot and Wladislav Mirzonov 2. The focus of activities will be put on small scale projects-projects with a well defined start and end. 3. Types of projects: Long stay for Professionals i.e. Troyka, Dwoyka-type lasting >10 days Professional/Senior/Management visits short stay < 7 days Repeat visits/further specialisation i.e. Grommets program 7-10 days 4. To explore options to participate in (Semi) Gouvernemental Projects i.e. in MATRA or in mini projects as mentioned by Mr.Raphael Varga van Kibed at the Dutch Embassy in Moscow (Feb 2009) Focal points: Wladislaw Mirzonov, Aleid Tensen and Edzard Braam 5. For 2009 two programmes are proposed: a Long Stay for 2 candidates and a short stay Management type visit for Doctors Mirzonov and Yanin. ( see below under “Double Dwoika” ) Both programmes in Emmen 6. For 2010 a Dwoika in Emmen, followed by DEMMA participation at a GP Symposium in Mytishchy, both in fall 2010

1-21 November 2009: DOUBLE DWOIKA – Medical Exchange Visit

Two young Medical Specialists, Dr. Alexander Aliev from Mytishchy Hospital and Dr. Alexander Lebedev, from Erisman Institute in Mytishchy, were invited by Demma for a three week stay in Emmen, Scheper Ziekenhuis. The programme was a great success non the least because of their enthousiasm and good knowledge of English.

18-21 November 2009: DOUBLE DWOIKA - Management Visit The newly appointed Medical head of Mytishchy hospital and Chairman of Demma-M Dr.Vladimir Mirzonov and Demma M secretary, Dr. Valery Yanin were offered the opportunity to familiarise themselves with aspects of the Dutch Health System. Management of Scheperziekenhuis hosted series of discussions and meetings on that issue. A short visit was paid to Mr.C.Bijl, Mayor of Emmen, who explained details of Private Hospital Management and Municipal/Regional Health systems.The visit was so succesfull that the two Russian guests were re-invited in 2010 for an extended orientational visit by Scheper Ziekenhuis Senior Management.

GP SYMPOSIUM , Mytishchi November 2009

On November 13th 2009 a General Practitioner Symposium was held in Mytishchi Hospital new facilities. Coen Holzhauer was invited to present a paper on the first line health care in the Netherlands. The audience was particularly interested in the financial structure of the General Practice. The main differences with Russia are the high percentage of insured persons and the phenomenon of subscription fees for the GP’s in The Netherlands. Another main difference is the principle in Russia of segregation between pediatric and adult medicine. A position for a family doctor is therefore not yet recognised as such. During the visit to Mytishchi a number of periferal GP stations were visited were substantial progress was made. Mytishchy has developed into a prosperous city and inhabitants, who can afford it are prepared to pay for high standards of health care. For the succesfull introduction of this high level of care, it will be nececcary in Russia to have accepted a physician at the front line with wide medical skills ( a so called specialised generalist) The presentation of Coen Holzhauer may have contributed to this development. The hospitality offered by Vladislav Mirzonov and Valery Yanin and many other medical colleagues was highly appreciated. Fortunately there was time left for visits to Khankhalaev Gallery and to learn more of the history of Kyrgyzstan.

FINANCIAL REPORT A Financial Audit Committee have approved the accounts over 2009. The possibilties for DEMMA to extend her activities in the near future will depend on the contributions of multiple sponsors. For further details please refer to the DEMMA Treasurer.
